• TB test past 12 months or CXR past 2 years
• State Exam: $175.00 – Payable to: Headmaster

Required Attire:

• Scrubs, closed-toed shoes and footies/socks are mandatory for all class sessions and clinical. Please come to class in proper attire or you will be asked to leave to change. 
• The school patch will be placed on the right sleeve. Hair needs to be off of the shoulders in a bun. NO LOOSE Ponytails.
• Minimal jewelry is preferable. NO HOOP EARRINGS.

• Please bring your phone, tablet, laptop, or any other appropriate electronic you have that is suitable for taking tests. If you do not have anything, a device will be provided to you at the time of testing.
